Overview
Bayada Home Health Care seeks a Per Diem Medical Social Worker, MSW to join our team providing home care visits in Bridgeport, Trumbull, Fairfield and surrounding towns. As a Medical Social Worker, MSW you will have the opportunity to help people in their homes by conducting initial assessments, developing and implementing a plan of care that specifies the client's problems and goals. LMSW required.
Responsibilities
Responsibilities include:
Make home visits to clients in designated geographic territories.
Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
Identify appropriate community resource referrals to address the client's and the family's practical and environmental needs.
Act as an advocate for both the client and the family, to navigate the patient through the community system.
Educate and instruct patients, family members, or other patient representatives, in community resources to promote the patient's health, safety, well-being and independent living.
Accurately document observations, interventions and evaluations pertaining to patient care management and services provided utilizing a state-of-the-art touch pad tablet.
Qualifications
Each Medical Social Worker must:
Have graduated from an approved school with a Masters Degree from a school of social work
Must be a Licensed Master's Social Worker (LMSW) in the state of Connecticut
Home health care experience is preferred
Have a minimum of one year experience in social work in a health care setting, and
Have a criminal background check completed.
